    Stem cells and metformin synergistically promote healing in experimentally induced cutaneous wound injury in diabetic rats

    Introduction. Diabetes mellitus (DM) is a serious, chronic metabolic disorder commonly complicated by diabetic foot ulcers with delayed healing. Metformin was found to have a wound healing effect through several mechanisms. The current study investigated the effect of both bone marrow-derived mesenchymal stem cells (BM-MSCs) and metformin, considered alone or combined, on the healing of an experimentally induced cutaneous wound injury in streptozotocin-induced diabetic rats.Material and methods. Forty adult male albino rats were used. Diabetes was induced by single intravenous (IV) injection of streptozotocin (STZ). Next, two circular full thickness skin wounds were created on the back of the animals, then randomly assigned into 4 groups, ten rats each. BM-MSCs were isolated from albino rats, 8 weeks of age and labeled by PKH26 before intradermal injection into rats of Group III and IV. Groups I (diabetic positive control), II (metformin-treated, 250 mg/kg/d), III (treated with 2×106 BM-MSCs), and IV (wounded rats treated both with metformin and BM-MSCs cells). Healing was assessed 3, 7, 14, and 21 days post wound induction through frequent measuring of wound diameters. Skin biopsies were obtained at the end of the experiment.Results. Gross evaluation of the physical healing of the wounds was done. Skin biopsies from the wound areas were processed for hematoxylin and eosin (H&E), Masson’s trichrome staining and immunohistochemical staining for CD31. The results showed better wound healing in the combined therapy group (IV) as compared to monotherapy groups.Conclusions. Although both metformin and BM-MSCs were effective in the healing of experimentally induced skin wounds in diabetic rats, the combination of both agents appears to be a better synergistic option for the treatment of diabetic wound injuries

    Effect of Detector Resolution on the Measurement of Nuclear Material Enrichment

    Nuclear material verification for safeguards purposes is an activity carried out to confirm that the amount of nuclear material (NM) present at a given time within a certain place is in agreement with the operator declarations. Nuclear materials are usually measured using gamma-ray spectrometer in order to quantify certain isotopes. Different codes have been used to quantify the isotopic abundance in nuclear material samples.  This study is investigating the performance of the Multi Group Analysis (MGA), the Multi Group Analysis for Uranium (MGAU) and the Full Range Analysis (PC/FRAM) at different energy resolution of the counting systems and different uranium isotopic compositions. The normalized measured/certified values (M/C) were used to monitor the performance of each code.  The performance of the three codes showed proportional relation to measured enrichments. PC/FRAM analysis provided the best consistency along the studied resolution and enrichment ranges with normalized measured/certified values ranging from 0 to 7%. MGAU and MGA showed more sensitivity towards low resolution detectors especially at lower enrichments with normalized measured/certified values ranged from 0 to 30 % and 0 to 20 % respectively

    Emerging relationship between vitamin D and LL-37 in the immune system’s response to infection and their possible role in combating sepsis

    Background: In patients who are critically unwell, sepsis could be one of major causes of mortality as well as many complications. Sepsis is the medical term describing dysfunctional body reaction to infection that results in dysfunctions in various body systems and hence severe organ failure could occur. A rise of 2 or more points in the sequential organ failure evaluation is an early indicator of organ dysfunction (SOFA score). Severe organ failure can occur if sepsis is left untreated. An infection-controlling effect of vitamin D has been demonstrated. With an increase in cathelicidin antimicrobial peptide (AMP) LL-37, human skin, blood plasma, monocytes, and macrophages are all affected. LL-37 and other AMPs, which operate directly against bacteria, regulate numerous innate and adaptive immunological functions. Objective: To make an overview of role of lack of vitamin D (25(OH) D) as well as AMP, LL-37, as risk factors for sepsis. Conclusion: Severe infections, as well as sepsis, have been linked to a deficiency of vitamin D. However, it is not yet clear what the underlying linkage and its clinical consequence is between sepsis and respiratory tract infections as well as critical illness

When humans are exposed to high levels of lead, there is damage to almost all organs and organ systems (most importantly, the central nervous system, kidneys, and blood), which often culminates in death.Objective: To estimate the prevalence of blood lead levels (BLLs) and to identify the sources of environmental exposure and potential risk factors for elevated BLLs among Saudi schoolchildren.Methods: A cross-sectional survey was conducted from March to May 2010. The study population included 557 Saudi school students of both sexes. A multistage random sampling technique was used. Information about socioeconomic status, house and school construction, and parents' education and employment was collected using questionnaires. Lead was analyzed in a blood sample using an atomic absorption technique and hemoglobin was measured using a Sysmex hematological analyzer.Results: The mean BLL was 4.94 ± 3.38 µg/dL (range 0.45–26.3 µg/dL). A total of 19% had BLLs <1.0 µg/dL, 16% had BLLs <2.5 µg/dL, 15% had BLLs <5.0 µg/dL, 20% had BLLs <7.5 µg/dL, 25% had BLLs <10.0 µg/dL, and about 6% had BLLs >10.0 µg/dL. Analysis of odds by controlling all risk factors (adjusted odds ratio [OR]) that affect BLLs (≥10 µg/dL) indicated that using cosmetics (OR = 18.5, confidence interval [CI] = 14.4–19.8), putting colored toys in mouth (OR = 15.7, CI = 3.6–16.2), eating canned food (OR = 9.8, CI = 7.0–10.1), and using newspaper during food preparation (OR = 7.6, CI = 6.3–8.2) are risk factors. There were significant correlations between BLLs and family habits (r = 0.225, P = 0.000), personal habits (r = 0.321, P = 0.000), eating habits (r = 0.128, P = 0.002) and school building characteristics (r = 0.469, P = 0.000). There was a significant correlation between BLLs and anemia in age group 6 < 12 years (P = 0.000) and age group 12 to less than 18 years, among males (P = 0.000) and females (P = 0.041).Conclusion: The BLLs of children are affected by multiple factors. Female students have higher BLLs and lower hemoglobin concentration than males. The possible sources of lead exposure were use of toothpaste, use of kohl, putting colored toys in the mouth, use of both canned food and canned juice, use of lip gloss in females, and different methods of handling newspaper while preparing food.Keywords: blood, lead, exposure, cosmetics, schoolchildren, anemi

    Photo-identification of Dugongs in Marsa Alam and Wadi El Gemal National Park, Egypt

    1351-1358Using photo-identification techniques, 30 dugongs were recorded at the southern Egyptian Red Sea coast between December 2015 and October 2017, 16 at Marsa Alam and 14 at Wadi El Gemal National Park (WGNP). Males were recorded seven times more frequently than females and calves were also recorded. A Photo ID catalogue was prepared for the dugongs with records of their occurrence among sites. We confirmed the presence of particular dugong specific sites. Long- and short-distance movements within the study sites were recorded for eight different dugongs. This is the first study to document the number of dugongs in inshore areas of the Egyptian Red Sea coast. Further studies are recommended for offshore sites in WGNP for better documentation of this group of animals

    The relationship between tissue transglutaminase IgA antibodies and the clinical manifestations in a group of children, adolescent and adult patients with type -I diabetes mellitus

    Background: Type-1 diabetes mellitus (T1-DM) is the commonest endocrine-metabolic disease in childhood. The prevalence of CD in type-1 DM ranges from 0.6 to 16.4% compared with 0.01–0.03% in the general population. The mechanism of association between the two diseases involves a shared genetic background of HLA genotype. Serum tissue transglutaminase IgA antibodies (tTG IgA) are considered specific and sensitive markers for screening of Celiac disease in more than 95 % of patients.Objective: Screening for the presence of serum tissue transglutaminase IgA antibodies (tTG ab) as a specific and sensitive biochemical marker for Celiac disease in patients with type-1DM and its relation to the clinical manifestations of those patients.Methods: One hundred-forty-nine patients with type-1 DM attending the out-patient clinic of endocrine and metabolism, Minia University Hospital were screened for the presence of serum tissue transglutaminase IgA antibodies during the period from March 2014 to November 2015.Results: Out of 149 patients 8 patients (5.3%) were positive for IgA tTG antibodies. They who were predominantly of female gender (75% were females). According to each age group, there were four sero-positive cases in children (with age group between 9 and ≤ 12 years); two cases in adolescents (with age group between 12 and ≤ 16 years) and two cases in adults (with age group 16-21 years). Intestinal manifestations, chronic diarrhea, recurrent abdominal pain/ distension, recurrent aphtha's stomatitis, anemia and bleeding tendency were significantly more common in sero-positive cases (P=0.001, 0.001, 0.016, 0.00, 0.001and 0.04 respectively). All sero-positive cases (100%) had lower BMIs than normal. There were no correlations between the tTG antibodies levels and HbA1c levels.Conclusions: The presence of tTG IgA antibodies is associated with significant changes in the clinical status of patient with type-1 DM. Celiac disease related manifestations like weight loss; anemia and chronic diarrhea were more common in sero-positive diabetic patients. Serological screening for CD should be performed in all patients with type-1DM for early diagnosis and prevention of complications.Keywords: Type-1 DM, tissue transglutaminase, IgA antibodie

    Trichorhinophalangeal syndrome II, expanding the clinical spectrum

    AbstractWe report a 4.5year old Egyptian male child, fourth in the order of birth of healthy remote consanguineous parents. He has typical facial as well as skeletal features of Trichorhinophalangeal syndrome (TRPS) II. The facial features included bilateral downward slanting palpebral fissures, bulbous nose, long filtrum, retromicrognathia, sparse hair in the scalp and thick eyebrows. The skeletal features included retarded bone age, cone shaped epiphyses of the phalanges and multiple exostoses. The patient has also growth retardation, moderate mental retardation and hyperlaxity of the right knee joint. However our patient has some features not reported in TRPS II patients. These included bilateral partial ptosis, long eye lashes, preauricular skin tag, short 2nd right finger, short metacarpals of both thumbs. So we have to expand the clinical spectrum. Karyotype demonstrated 46,XY,del 8(q23.3-q24.1)
